How to Find Health Care
This list is intended to help those who are low-income, uninsured, or receiving
government assistance and are in need of primary health care.
Choosing the right clinic can be difficult. Review the following questions, or
call 2-1-1 for help locating the most appropriate resource.
What is the nature of the medical need?
Listed clinics provide services that meet differing types of medical needs. Most
provide primary care, and offer: preventative care (immunizations, annual
checkups/physicals), care for chronic conditions (diabetes, asthma), and acute
care (colds, sore throats, flu). Some clinics provide specific or specialized
services, and would not be appropriate for all needs. To identify if a clinic
provides a specific service only, or to identify which type of service the clinic
offers, see the clinic’s description, “Services,” or “Eligibility Requirements.”
Is the individual currently insured?
The clinics listed here provide medical services to individuals or families that
are uninsured or participate in government funded insurance programs (i.e.
Medicaid, Medicare, Veterans, Primary Care Network (PCN), etc.). Individuals
with insurance should call the insurance carrier to locate a provider.
Has the individual been to a clinic before?
It is advisable to continue services at the same clinic, if available or
appropriate. If the individual’s financial or insurance situation has changed,
consider first discussing this issue with his/her previous clinic. Some clinics,
due to specialized purposes, capacity, or budget constraints do not accept
certain insurance providers or new clients. To identify a clinic’s limitations or
availability to accept patients or insurance see “Eligibility Requirements”
and/or “Area Served.”
Does the individual live within a specific geographic area?
Some clinics serve only patients living within specific boundaries, such as a city
or zip code, and will not serve individuals outside of those boundaries. To
identify if a clinic has boundary limitations, see the clinic’s “Fees,” “Eligibility
Requirements,” and/or “Area Served.”
Does the individual belong to a specific population?
Some clinics serve only patients of a specific population, such as veterans, the
homeless, or Native Americans. If the individual does not belong to that
population, the clinic will not serve him/her. To identify if a clinic works only
with a specific population, see the clinic’s “Eligibility Requirements.”
Key terms:
Urgent care: untreated acute or chronic cconditions that need to be addressed
quickly, but are not life threatening. Emergency care: services for lifethreatening
needs ( heart attack, stroke, etc.). Specialty care: services for
specific medical conditions (neurology, surgery, etc.). In most cases a patient
needs to be referred to a specialist by his/her primary care physician.
For a listing of government-funded insurance programs, please see the Public
Health Insurance Resource List.
