Caring Foundation for Children
A Program of Regence Blue Cross Blue Shield of Utah.
Provides dental benefits for uninsured children at no cost to
their families. The program generally includes one-year
coverage up to $1,000 for dental work including: exams, xrays,
cleanings, fluoride, sealants, resins, extractions,
crowns, space maintainers, fillings, and root canals.

Eligibility Requirements: Open to children, documented
or not, ages 0-18, living in Utah and Idaho, who are not
eligible for Medicaid or CHIP (if CHIP enrollment is closed
the children are eligible), and whose family income is at or
below 200% of the federal poverty line.
CHIP
(Children’s Health Insurance Program)
A program of the Utah Department of Health
Provides comprehensive health insurance, including limited
dental, to Utah children who qualify.

Eligibility is determined by the Department of Workforce
Services (DWS) or the Bureau of Eligibility Services (BES).

Public Employees Dental Program (PEDP): 366-7555

Fees: Quarterly premiums and co-payments for some services
Eligibility Requirements: U.S. citizen or legal resident,
under age 19, at or below 200% of federal poverty income
guidelines, and not covered by any other health insurance.
Families must apply during open enrollment.
Medicaid
A program of the Utah Department of Health
Provides health coverage for Utah adults and children who
qualify. Covered services vary according to type of Medicaid,
see contact information below.

Services (DWS) or the Bureau of Eligibility Services (BES).

Eligibility Requirements: Varies according to type of
Medicaid. Factors such as age, income and assets, medical
condition, etc. can affect eligibility. Contact an Eligibility

PCN (Primary Care Network)
A program of the Utah Department of Health
Provides health, and limited dental coverage for Utah adults
who qualify. Dental services include: examinations, x-rays,
cleanings, and fillings.

Services (DWS) or the Bureau of Eligibility Services (BES).

Fees: 10 % co-pay of allowed amount
Eligibility Requirements: Age 19 - 64, U.S. citizen or
legal resident, fit within the PCN income guidelines (150%
of federal poverty income guideline), and do not have access
to student health insurance, Medicaid, Medicare, Veterans
benefits, or health insurance. Individuals must apply during
